Important Policy Information

Please view the following important information regarding your policy

  • Members can retain coverage even if they are furloughed or suspended
  • Policy holders must contact the customer Service Center at (224-770-5328) or info@smart-vltd.com immediately if there is an interruption of work status as a result of furloughs, injury, illness, discipline or if you retire or leave the union
  • Policy holders must continue to pay monthly premiums regardless of work status to maintain their coverage
  • Policy effective dates are determined by Amalgamated Life Insurance Company and are not the date of application
  • Applicants must be actively working on the effective date for coverage to begin
